Clearing Judgment?

Over 8 years ago, my husband and I satisfied a judgment that was showing up on the title of our home. Everytime we do anything with the house (re-finance, 2nd mortgage, etc.), it always comes back up again. When we did a re-fi around 4 years ago, we even took the 'paid in full' paper work down to the courthouse and made sure they had a copy (it even shows it was filed in LA County).... yet .... we're doing some financial stuff & it's back!!! What's it take to get it to go away???
